YJFL Code of Conduct (for each player registering)

Medical Profile (for each player registering)

Tackers Friendship Group (for each player turning 9 or 10 this year)

YJFL Registration Form (for each player registering)

Clearance Form (for new players turning 16 or 17 this year, who have played football in the past 3 years for any other club)

The Club will supply personalised forrms,
pre-completed with last year's information to all previous players. Please use those supplied forms rather than completing new ones from here.

Coaches

Adults who will coach or be assistant coaches for the first time with the Sharks must complete a
Coaches Registration Form.
